										<content:encoded><![CDATA[<p style="text-align: justify;">Rotator cuff tear is a common cause of shoulder pain and disability among adults and the elderly. Many daily activities, like combing your hair or getting dressed, may become painful and difficult to do and the patient may have severe pain at night affecting sleep.</p>
<p style="text-align: justify;"><strong>The Shoulder Joint</strong></p>
<p style="text-align: justify;">The shoulder complex is made up of three bones: your upper arm bone (humerus), your shoulder blade (scapula), and your collarbone (clavicle). The shoulder joint is a ball-and-socket type of joint: The ball, or head, of your upper arm bone, fits into a shallow socket in your shoulder blade made of scapula bone.</p>
<p style="text-align: justify;">The Head of the Humerus (Arm Bone) is kept in your shoulder socket by your rotator cuff. The rotator cuff of the shoulder joint is a group of four muscles that come together as tendons to form a covering around the head of the humerus. The rotator cuff attaches the humerus to the shoulder blade and helps to lift and rotate your arm around the joint.</p>
<p style="text-align: justify;">There is a sac called a bursa between the rotator cuff and the bone on top of your shoulder (acromion) that provide lubrication in the joint. The bursa is the Subacromial bursa, which allows the rotator cuff tendons to glide freely when you move your arm. When the rotator cuff tendons are injured or torn, this bursa can also become inflamed and painful.</p>

<p style="text-align: justify;"><strong>The four muscles rotator cuff tendons are:</strong></p>
<ul style="text-align: justify;">
<li><strong>Supraspinatus:</strong> It is there at the top of the shoulder.  Initiates abduction i.e. moving the arm out sideways, and stabilises the shoulder during this movement.  Most commonly injured tendon.</li>
<li><strong>Infraspinatus:</strong> It is at the back of the shoulder.  It rotates the arm outwards.</li>
<li><strong>Teres Minor:</strong> It is at the back of the shoulder below the infraspinatus. It also turns it outwards.</li>
<li><strong>Subscapularis:</strong> This runs across to the front of the shoulder.  It medially rotates the arm i.e. turns it inwards</li>
</ul>
<p style="text-align: justify;"><strong> Rotator cuff tears</strong></p>
<p style="text-align: justify;">Most tears occur in the supraspinatus tendon, but other parts of the rotator cuff may also be damaged.</p>

<p style="text-align: justify;">In most cases, tears in tendons begin by fraying. As the injury progresses, the tendon can completely tear.</p>
<p style="text-align: justify;"><strong>There are different types of tears.</strong></p>
<ul style="text-align: justify;">
<li><strong>Partial tear.</strong> this is also called an incomplete tear. It damages the tendon but does not completely damage it.</li>
<li><strong>Full-thickness tear.</strong> also called a complete tear. It separates all of the tendons completely from the bone.</li>
</ul>
<p style="text-align: justify;"><strong>Cause of Rotator cuff tears</strong><br />
There are two main causes of rotator cuff tears: injury and degeneration.</p>
<ul style="text-align: justify;">
<li><strong>Acute Tear</strong><br />
If fall down on your outstretched arm at home or road traffic accident or lift something too heavy with a jerking motion, your rotator cuff might be torn.</li>
<li><strong>Degenerative Tear</strong><br />
These types of tears are the result of age-related wearing down of the tendon that occurs slowly over time. These are more common in the dominant shoulder. If you are having a degenerative tear in one of the shoulders, there is a more chance of a rotator cuff tear in the opposite shoulder — even if you do not have pain in that shoulder.</li>
</ul>
<p style="text-align: justify;"><strong>These are factors that contribute to degenerative, or chronic, tears.</strong></p>
<ul style="text-align: justify;">
<li><strong>Repetitive stress.</strong> Repeating the same shoulder motions, again and again, lead to stress on your rotator cuff muscles and tendons. Example Baseball, tennis, rowing, and weightlifting. These sports activities can put you at risk for overuse tears.</li>
<li><strong>Lack of blood supply.</strong> With increasing age, the blood supply in our rotator cuff tendons lessens. This can ultimately lead to a rotator cuff tendon tear.</li>
<li><strong>Bone spurs.</strong> With ageing bone overgrowth often develop on the underside of the acromion bone. When you lift your shoulder, this bony outgrowth rub on the rotator cuff tendon.</li>
</ul>
<p style="text-align: justify;"><strong>Risk Factors</strong></p>
<p style="text-align: justify;">Most rotator cuff tears are caused by the normal wear and tear that goes with ageing, people over 45 are at greater risk.</p>
<p style="text-align: justify;">People who do repetitive overhead activities or lifting are also at risk for tears. Overhead Athletes are likely to overuse tears, particularly tennis players and baseball pitchers.</p>
<p style="text-align: justify;">Most tears in young adults are caused by a traumatic injury, like a fall.</p>

<p style="text-align: justify;"><strong>Torn Rotator Cuff Symptoms</strong></p>
<p style="text-align: justify;">Degenerative torn rotator cuff tear symptoms at the start are fairly mild and get progressively increase over time.</p>
<p style="text-align: justify;">Acute rotator cuff tears more typically cause instant, intense pain.</p>
<p style="text-align: justify;"><strong>Torn rotator cuff symptoms commonly are:</strong></p>
<ul style="text-align: justify;">
<li><strong>Pain:</strong> around the shoulder, often is as a dull ache deep inside. Pain tends to get worse when lifting or rotating your arm, with activities such as brushing your hair. Pain is worse when lying on that side in bed, which often leads to disturbed sleep at night.  As the condition gets worse, you may experience pain even on rest.</li>
<li><strong>Weakness:</strong> Difficulty lifting or twisting the arm is common which can lead to decreased range of active movement.</li>
</ul>
<p style="text-align: justify;">Symptoms will depend on the severity of the injury. Three grades for tendon strains are:</p>
<ul style="text-align: justify;">
<li><strong>Grade 1 Strain:</strong>  No fibres are torn,  but overstretching of the tendon</li>
<li><strong>Grade 2 Strain:</strong> a partial rotator cuff tear – only some of the fibres are torn</li>
<li><strong>Grade 3 Strain:</strong> a complete/full-thickness rotator cuff tear &#8211; the tendon completely detaches from the bone itself. Due to this, you may not be able to lift your arm.</li>
</ul>
<p style="text-align: justify;"><strong>Diagnosing Rotator Cuff Tears</strong></p>
<p style="text-align: justify;">After taking your history doctor will examine you for your problems. It is always advisable to visit a shoulder specialist arthroscopy orthopedic surgeon.</p>

<p>Your hip is a ball and socket type joint. The socket is made by the acetabulum, which is part of the large pelvic bone. The ball of joint is the femoral head, which is the upper end of your thighbone.</p>
<p>The surface of the ball and socket is covered with cartilage, a smooth, slippery substance that protects the bones and enables them to glide smoothly across each other.</p>

<p><strong>MRI SCAN</strong></p>
<p>Early changes in the bone that may not show up on an X-ray can be detected with an MRI scan.</p>
<p>This is used to evaluate how much of the bone is affected by the disease.</p>
<p>An MRI may also show early AVN that has yet to cause symptoms (for example — AVN that may be developing in the opposite hip joint).</p>

<p class=" eplus-wrapper">The knee joint is formed by the lower end of femur, upper end of tibia (leg bone) and patella (knee cap). The ligaments of the knee play a crucial role as they are the most important element in providing stability to the knee.</p>



<p class=" eplus-wrapper"><strong>Ligaments of the Knee</strong></p>



<p class=" eplus-wrapper">The knee is stabilized by four main ligaments &#8211; two ligaments in the center of the joint (cruciate) and two ligaments on either side of the knee (collateral ligaments).The Anterior Cruciate Ligament (ACL) is in the front of the knee joint and it prevents the tibia from slipping forward. The Posterior Cruciate Ligament (PCL) is at the back of the knee and it prevents the tibia from slipping backwards.</p>



<p class=" eplus-wrapper">The collateral ligaments prevent abnormal sideward movements of the tibia. The Medial Collateral Ligament (MCL) stabilizes the inner side of the knee and the Lateral Collateral Ligament (LCL) along with a few more ligaments stabilizes the outer aspect of the knee.</p>

<p class=" eplus-wrapper">Many people can hear a “CRACK” or “POP” at the time of injury, followed by immediate swelling and pain. Players often have to be carried off the field. Later there will be pain, weakness and a peculiar looseness of knee- giving way- the most disabling problem.</p>



<p class=" eplus-wrapper"><strong>What is the treatment</strong></p>



<p class=" eplus-wrapper">It depends on your disability and your activities. In brief, if you are engaged in lots of outdoor activities and heavy work and not willing to restrict your activities then you need an operation to make a new ligament. This applies to most young and active people.</p>



<p class=" eplus-wrapper"><strong>What is the operation</strong></p>



<p class=" eplus-wrapper">Operation is done in general or spinal anesthesia. At first inside of knee is examined by a telescope, which is introduced through small puncture (arthroscopy). Next a new ligament is prepared by taking few fibers from the tendons around knee. Generally the hamstring or patellar tendon is used.</p>



<p class=" eplus-wrapper">This newly fashioned ligament is then placed inside the knee which grows and matures to take the function of original ligament.</p>



<p class=" eplus-wrapper"><strong>What if I do not get operated</strong></p>



<p class=" eplus-wrapper">Other then the disabilities like giving way, pain, swelling etc. it is said to be the “Beginning of The End of Knee.” The knee suffers repeated injuries because of its undue laxity and early arthritis sets in.</p>

<p class=" eplus-wrapper"><strong>ACL Surgery Guideline For patients</strong></p>



<p class=" eplus-wrapper"><strong>Preparation before the operation:</strong></p>



<p class=" eplus-wrapper">1. Arrange for a leave about one month from your work.</p>



<p class=" eplus-wrapper">2. DO NOT eat or drink anything after Midnight. (NBM)</p>



<p class=" eplus-wrapper">3. Report to hospital 8 a.m. – Please bring your all X ray and reports with you.</p>



<p class=" eplus-wrapper">4. The hospital stay is usually 2-3 days – bring your clothes accordingly.</p>



<p class=" eplus-wrapper"><strong>After operation</strong></p>



<p class=" eplus-wrapper"><strong>1. Exercise:</strong> Start the static quads exercise the same day. From next day a qualified physiotherapist will take you through a detailed exercise schedule.</p>



<p class=" eplus-wrapper"><strong>2 .Walking: </strong> You can stand up next morning and start walking. You will need the help of a stick or a pair of crutches for the first few weeks. However, walk minimally for first two weeks after operation.  This reduces pain, swelling and helps healing.</p>



<p class=" eplus-wrapper"><strong>3. ICE:</strong> Compress: Put some crushed ice in a plastic bag, cover it with dry towel and place it on the knee for 15-30 minutes. Do it four times daily.</p>



<p class=" eplus-wrapper"><strong>4. Dressing:</strong> Will be changed in the hospital the after the operation. No further dressing is usually necessary. Keep it dry until stitches are removed.</p>



<p class=" eplus-wrapper"><strong>5. Stitch removal:</strong>  Usually done 12-14 day after operation.</p>



<p class=" eplus-wrapper"><strong>6. Shower:</strong> Allowed only after 2 weeks, when stitches are removed.</p>



<p class=" eplus-wrapper"><strong>7. Toilet:</strong> You should use commode for 2-3 months after operation (If you do not have commode at home arrange for a chair commode)</p>



<p class=" eplus-wrapper"><strong>8. Driving:</strong>  You can drive a car after two months and a two wheeler after three months, provided you have good power and control of leg.</p>



<p class=" eplus-wrapper"><strong>9. Activities:</strong> During first two weeks you should mostly rest in bed with leg elevated on a pillow, and do exercise as advised. Subsequent movements depend on your muscle strength and type of operation.</p>



<p class=" eplus-wrapper"><strong>Generally, after 4 weeks one can join light office duties.</strong></p>



<p class=" eplus-wrapper"><strong>After 8 weeks one can join gym, ride a bicycle or drive a car.</strong></p>



<p class=" eplus-wrapper"><strong>After 4 months light on field activities are allowed.</strong></p>



<p class=" eplus-wrapper"><strong>After 6 to 9 months games can be played provided good muscle power is achieved.</strong></p>

<p class="eplus-u1NFi2">The shoulder is made up of three bones: upper arm bone (humerus), shoulder blade (scapula), and collarbone (clavicle).</p>



<p class="eplus-W25zXa">The head, or ball, of upper arm bone fits into a shallow socket in your shoulder blade (Glenoid). Strong connective soft tissue, called the shoulder joint&nbsp;capsule, is the ligament system of the shoulder and keeps the ball&nbsp;centered in the glenoid socket.&nbsp;</p>



<p class="eplus-oNhyS4">Strong tendons and muscles around the joint&nbsp;keeps your shoulder joint stable.</p>



<h2 class="eplus-kx4IAH wp-block-heading"><strong><em>Shoulder Dislocation</em></strong></h2>



<p class="eplus-S5aF2g">Sports injury, or accidental trauma, is often the cause of an initial shoulder dislocation. When the head of the humerus dislocates, the socket bone (glenoid) and the ligaments and capsule in the front of the shoulder are often damaged. The labrum — the soft tissue&nbsp;rim around the edge of the glenoid — may also tear. This is commonly called Bankart lesion. A severe first dislocation can lead to continued dislocations, giving out, or a feeling of instability. Young patients are more likely to get repeat dislocations.</p>

<p class="eplus-ZEmbbG">When the sling is removed, exercises to rehabilitate shoulder&nbsp;will be started. These will improve the range of motion in&nbsp;&nbsp;shoulder and prevent scarring as the ligaments heal. Exercises to strengthen your shoulder will gradually be added to your rehabilitation.</p>

<h1 class="wp-block-heading"><strong>The Shoulder Joint </strong></h1>



<p>It is ball-and-socket joint type joint. The bone in your upper arm (humerus) has a round end that acts as a ball and  fits into the curved structure (socket) on the outside of your shoulder blade.</p>



<p>Ligaments, tendons and muscles hold it together. A layer of tissue, cartilage keeps the bones apart, so they don&#8217;t rub against each other.</p>



<p>The ball and socket type joint lets you move your arm up and down, back and forward, or in a circle.</p>

<h2 class="wp-block-heading"><strong>What is done in surgery </strong></h2>



<p>Shoulder surgeon will replace the bone in the ball and socket of your shoulder joint with a material that could be metal and plastic. You will need physical therapy the surgery.</p>



<p>There are three types of shoulder replacement surgeries:</p>



<p><strong><em>Total shoulder replacement:</em></strong> This is the most common type shoulder replacement. Here the surgeon replaces the ball at the top of your humerus with a metal ball, which gets attached to the remaining bone. The socket of the joint gets covered with a new plastic surface.</p>



<p><strong>Hemi shoulder replacement:</strong> Only the ball gets replaced at the top of your humerus with a metal ball.</p>



<p><strong>Reverse shoulder replacement</strong>:</p>



<p>This is done mostly if they have a torn rotator cuff with arthropathy.</p>



<p>Nowadays due to better results also done in case of fracture around shoulder with shattered bone.  </p>



<p>It’s also done when another shoulder replacement surgery didn’t work.</p>

<h2 class="wp-block-heading"><strong>Preparing for Your Surgery</strong></h2>



<p>Your Shoulder surgeon will do a full examination of your shoulder. X-rays, CT scan MRI or other imaging tests will be required.</p>



<p>Talk to your doctor about any other  medical conditions you have and any medications you take. You will have to stop taking certain drugs, especially painkillers, several weeks before surgery</p>

<h2 class="wp-block-heading"><strong>Recovery</strong></h2>



<p>You can expect to spend 2 to 4 days in the hospital after your operation.  You will be given pain medicine to reduce pain. Cold compression can help control the swelling.</p>



<p>At first, your arm will be kept in a brace to give rest. Within a few days your doctor will  start physical therapy to get your arm and your new shoulder started working.</p>



<p>You will do exercises that will gradually improve how your new joint works.</p>



<p><strong><em>Don’t rush things:</em></strong></p>



<p>It may take 4 to 6 weeks before you can pick up anything heavier than a glass of water .</p>



<p>Your arm will be in an arm sling for most of your recovery. It may be 8 to 10 weeks or more before you can drive again.</p>



<p>You will go for several follow-up visits with your doctor so they can see how your recovery is going.</p>

<h2 class="wp-block-heading"><strong>Complications</strong></h2>



<p>Most people have full motion in their shoulder and return to their daily routine after shoulder replacement surgery. But as with any operation, there is a chance of complications.</p>



<p><strong>Infection or bleeding: </strong>You will be given antibiotics after your surgery to lower your chances of infection.</p>



<p><strong>Dislocation: </strong>This happens when the ball slips out of the socket.</p>



<p><strong>Nerves around your shoulder may get damaged: </strong>They usually recover with time.</p>



<p><strong>After years of use, the replacement joint may become loose or separate from the bone</strong>: You may need surgery to fix it.</p>

<p>अगर किसी व्यक्ति के लिए खेल में लगी चोट के तुरंत के बाद आराम करना संभव नहीं है, तो वह इसके लिए बर्फ के टूकड़े का इस्तेमाल भी कर सकता है।</p>



<p>बर्फ का टुकड़े का इस्तेमाल करने से चोट वाली जगह पर सूजन होने की संभावना, ब्लड फ्लो, थोड़ी-थोड़ी देर में होने वाले दर्द इत्यादि कम हो जाते हैं।</p>

<p>अक्सर, किसी खिलाड़ी को शरीर के निचले हिस्से (जैसे-पैर) इत्यादि पर चोट लगती है, तो उसका इलाज चोट वाली जगह को उठाकर किया जा सकता है। ऐसा करने से खिलाड़ी को आराम मिलता है और वह जल्दी ठीक हो जाता है।</p>
